Our patient information leaflets provide guidance and advice to patients, carers and visitors on a number of services that we deliver.

Each leaflet is reviewed every three years so that readers have up to date information.

Advance Care

Planning (ACP) information

 

This leaflet explains how to consider your choices and preferences for the future. Our Hospital Specialist Palliative Care Team is able to support you with this, if you need help. 

 

 

What is advance care planning (ACP)?

   

 

ACP is the process of deciding what plans you want to make for your future care, often in discussion with a healthcare professional. This may take place when you are thinking about what care you might need in the future and what things you may wish to avoid in the future.

Sometimes, you might choose to talk through your choices and wishes when you are well enough to do so.  It may be difficult to talk together with your family as you may not always agree, but having these conversations can help direct decisions that sometimes need to be made at a time of crisis.

What does the Advance Care Plan include?

 

This can cover any aspect of your future health or social care and can include anything that’s important to you. Examples of things you may like to include in your plan are:

 

  • Who you would like to make any decisions if there are choices to be made about your care?
  • Where you would like to be cared for if/when you become too unwell?
  • Any practical issues you have concerns about, such as who will look after your pet if you become ill.

 

Our Hospital Specialist Palliative Care Team can guide and help with this.  We could also share these decisions with your health care professionals if you would like us to, using an electronic system like Co-ordinate My Care (CMC).

 

 

Can I change my mind?

 

You can change your mind about anything you put in your ACP at any time as you wish. If you do change your mind about something, let your healthcare professionals know, and we will ensure that any changes are written down, so everyone involved in your care knows your preferences.

9am – 5pm.  While you are in hospital, you can ask your doctor or nurse to call us on your behalf or contact us directly if you prefer.

 

Your relatives and friends can also contact us if they wish.
